Personalized Request forms

Each client is given personalized request forms pre-printed with the clinic account name and number. Plain forms, check box forms, and personalized check-up forms are available, so please specify your preference when ordering. The bottom copy is for your records.

• Please write the patient’s name, age or date of birth, sex, and HKID or clinic number (optional) clearly on the request form.
• Be sure to label all the specimen containers.
• Place the specimen(s) and the matching request form(s) together into a small plastic bag.
• Check the courier schedule on the specimen pickup web page
• Phone our laboratory for pickup service.

Qualified medical professionals with an account number may print out these PDF forms for use. Please print our new accounts form if you do not have one yet.
